Church Assistance Application
The applicant completes and signs only the public sections below. Pastor and authorized Prayer & Care staff review and complete the internal decision, disbursement, follow-up, and closure sections.
Enter the information of the person who will sign this request. When an HSM staff member is only helping with intake, the staff member does not replace the requester. The system records the assisting staff member privately through staff login.

Submit prayer needs, intercession needs, or anointing service requests for yourself, your family, or someone you are standing with in faith.
Anointing Service
Request anointing prayer, altar care, consecration prayer, or spiritual support through the Prayer Ministry.
Home & Hospital Communion
Home and hospital communion is available to all members by request and upon request of their families. Holy Communion Sunday is a separate worship event every 1st Sunday.
